Evaluation Visit Update Report


Prior to any evaluation visit, an institution should forward the Evaluation Visit Update Form directly to each team member at least ten (10) days prior to the scheduled visit. Institutions with multiple locations must complete a separate report for the main campus and for each branch campus.

The Evaluation Visit Update Form consists of an update of the current enrollment by full-time and part-time status and by day and evening, a class schedule that will be in effect on the day or days of the visit, a copy of the most recent catalog, a list of the faculty and administrative staff hired since the submission of the self-study, a current Institutional Effectiveness Plan, and a description of all the significant changes that have occurred since the submission of the application materials.
